Dentistry intake forms: FAQ
What intake forms does a dental practice use?
Most start with a new patient form and a dental medical history form. Specialist and procedure forms follow: orthodontic, implant, oral surgery, sleep apnea, and TMJ questionnaires. Returning patients often fill in a short health history update instead of the full form.
Is it the same as a dental medical history form?
Not quite. The intake form, or new patient form, collects contact, insurance, and visit details. The medical history form covers dental history, medications, allergies, conditions, and past surgeries. Many practices hand out both at the first visit.
Who fills in dental intake forms?
Adult patients fill in their own forms. For a child, a parent or legal guardian answers the history questions and signs the consent. A family member or caregiver often helps patients with special needs.
How are pediatric dental forms different?
A parent answers for the child. The forms ask about birth and growth, feeding and brushing habits, thumb sucking, and how past visits went. The ADA's children's health history form also asks about vaccination status. Sedation forms add questions about breathing and recent illness.
How does AI Doctor help a dental practice?
AI Doctor, a real-time AI healthcare assistant avatar, asks the intake questions as a conversation, in the patient's own language. It collects the answers for the dental team to review before the visit.
Which form should a new practice start with?
Start with the new patient form and the dental medical history form, because every patient needs them. Add specialist forms, such as orthodontic or implant consultation forms, as you offer those treatments.
Does AI Doctor give medical advice?
No. It collects information for the care team. It doesn't diagnose, triage, or recommend treatment.
Can patient answers stay on our own servers?
Yes. RAVATAR offers on-prem deployment: the platform can run inside your own perimeter, so patient answers stay there. RAVATAR complies with GDPR and the EU AI Act.
