Dermatology intake forms: FAQ

What intake forms does a dermatology practice use?

Most practices start with a new patient form and a medical history. Then come questionnaires for the condition being treated, such as acne, eczema or psoriasis, plus forms for skin checks, Mohs surgery and cosmetic treatments.

What does a dermatology intake form ask?

It asks about the skin problem: where it is, when it started, how it has changed and what has been tried. It also covers medicines, allergies, sun exposure, past skin cancers and family history of skin disease.

Why do cosmetic treatments have separate forms?

Cosmetic visits ask different questions from medical ones: the patient's goals, past procedures, skin type, recent sun exposure and medicines that affect healing. Laser and tattoo removal forms also check for anything that makes a treatment unsuitable.

What is the DLQI?

The Dermatology Life Quality Index is a ten-question form for patients aged 16 and over about how a skin condition affects daily life, work, relationships and treatment. Each answer is scored, giving a total from 0 to 30, and practices repeat it to track change over time.

How does AI Doctor help a dermatology practice?

AI Doctor, a real-time AI healthcare assistant avatar, asks the intake questions as a conversation, in the patient's own language. It collects the answers for the team to review before the visit, so the visit starts with the history already in place.

Which form should a new dermatology practice start with?

Start with the dermatology new patient form, since every patient needs it. Add questionnaires for the conditions you see most, such as acne or eczema, then skin cancer and cosmetic forms if you offer those services.

Does AI Doctor give medical advice?

No. It collects information for the care team. It doesn't diagnose, triage, or recommend treatment.

Can patient answers stay on our own servers?

Yes. RAVATAR offers on-prem deployment: the platform can run inside your own perimeter, so patient answers stay there. RAVATAR complies with GDPR and the EU AI Act.
