Gastroenterology intake forms: FAQ

What intake forms does a gastroenterology practice use?

Most start with a new patient questionnaire: current digestive symptoms, past GI problems and procedures, medicines, allergies, and family history. Procedure questionnaires prepare patients for colonoscopy or upper endoscopy. Condition forms then go deeper on reflux, IBS, IBD, liver disease, or pancreatitis.

What does a colonoscopy questionnaire ask?

It usually asks why the procedure is planned, past colonoscopies and polyps, family history of colon cancer, current symptoms, medicines such as blood thinners and diabetes drugs, allergies, and sedation history. Each practice sets its own prep instructions, so the form complements them rather than replacing them.

Why do GI forms ask about family history?

Some digestive conditions, including colon cancer, IBD, and celiac disease, can run in families. Family history also affects when and how often a person is advised to have screening, so most GI forms ask about it.

What is the GerdQ questionnaire?

GerdQ is a six-question tool about reflux symptoms over the past week, such as heartburn and regurgitation, plus sleep disturbance and extra medicine use. Answers are scored and added up. It is a proprietary instrument, so practices check the licensing terms before using it.

How are pediatric GI forms different?

A parent or guardian answers. The forms ask about growth, feeding and diet, toilet habits, and how symptoms affect school and sleep. Child constipation forms often ask about stool patterns and toilet training in more detail than adult forms.

How does AI Doctor help a gastroenterology practice?

AI Doctor, a real-time AI healthcare assistant avatar, asks the intake questions as a conversation, in the patient's own language, and collects the answers for the team to review before the visit. It does not diagnose, triage or give medical advice.

Does AI Doctor give medical advice?

No. It collects information for the care team. It doesn't diagnose, triage, or recommend treatment.

Can patient answers stay on our own servers?

Yes. RAVATAR offers on-prem deployment: the platform can run inside your own perimeter, so patient answers stay there. RAVATAR complies with GDPR and the EU AI Act.
