Neurology intake forms: FAQ
What intake forms does a neurology practice use?
Most start with a neurology new patient form: the main symptom, when it started, how it has changed, medical history, and medications. Subspecialty clinics add their own questionnaires, such as headache diaries and scales, seizure histories, or memory clinic forms.
Is a neurology intake form the same as a general medical history?
It covers the same basics, but asks much more about the symptom itself: where it is, how long it lasts, what triggers it, and what makes it better or worse. Neurologists rely on this history to decide which exam and tests come next.
What do the MIDAS and HIT-6 headache questionnaires measure?
Both measure how much headaches affect daily life. MIDAS counts days of missed or reduced work, home, and social activity over the past three months. HIT-6 has six questions about headache impact over the past four weeks. Clinics often use them to track change over time.
Why do memory clinics ask a caregiver to fill in a form?
People with memory problems may not notice or recall changes in themselves. A family member or caregiver can describe what has changed, when it started, and how daily life is affected. The clinic reads both views together.
How does AI Doctor help a neurology practice?
AI Doctor, a real-time AI healthcare assistant avatar, asks the intake questions as a conversation, in the patient's own language, and collects the answers for the team to review. The neurologist sees the history before the visit.
Which form should a new practice start with?
Start with the neurology new patient form, which every visit needs. Then add the questionnaires for the conditions you see most, such as the headache new patient questionnaire or the epilepsy new patient questionnaire, and the follow-up forms for return visits.
Does AI Doctor give medical advice?
No. It collects information for the care team. It doesn't diagnose, triage, or recommend treatment.
Can patient answers stay on our own servers?
Yes. RAVATAR offers on-prem deployment: the platform can run inside your own perimeter, so patient answers stay there. RAVATAR complies with GDPR and the EU AI Act.
