OB/GYN intake forms: FAQ

What intake forms does an OB/GYN practice use?

Most start with a new patient form covering periods, past pregnancies, birth control, screening history, and general health. Pregnant patients fill in a new OB form. Gynecology clinics add questionnaires for specific concerns, such as pelvic pain, heavy periods, or menopause symptoms.

How is a new OB form different from a gynecology form?

A new OB form is about the pregnancy: the last period, past pregnancies and births, family and genetic history, and health risks that affect prenatal care. A gynecology form focuses on periods, birth control, sexual health, and screening history.

What is the Edinburgh Postnatal Depression Scale?

A 10-item questionnaire about how the patient has felt over the past week. It is used during pregnancy and after birth to screen for perinatal depression. A score is a reason to talk further, not a diagnosis.

Who fills in adolescent gynecology forms?

Usually the teen and a parent or guardian together, with the parent adding family and medical history. Which questions a teen answers on her own varies by practice and by state rules on confidential care.

How does AI Doctor help an OB/GYN practice?

AI Doctor, a real-time AI healthcare assistant avatar, asks the intake questions as a conversation, in the patient's own language, and collects the answers for the team to review. Sensitive questions can be asked in a calm, private format before the visit.

Which form should a new practice start with?

Start with the OB/GYN new patient intake form, which covers both sides of the practice. Add the new OB patient intake form if you provide prenatal care, then the questionnaires for the concerns you see most.

Does AI Doctor give medical advice?

No. It collects information for the care team. It doesn't diagnose, triage, or recommend treatment.

Can patient answers stay on our own servers?

Yes. RAVATAR offers on-prem deployment: the platform can run inside your own perimeter, so patient answers stay there. RAVATAR complies with GDPR and the EU AI Act.
