Orthopedics intake forms: FAQ

What intake forms does an orthopedic practice use?

Most practices start with a new patient form, a medical history form and a pain diagram. Then come forms for the body part being treated, such as the knee, hip, shoulder or spine, plus questionnaires before and after surgery.

Is an orthopedic intake form the same as a medical history form?

Not quite. The intake form covers the visit: the problem, when it started, how it happened and what makes it worse. The medical history form covers past conditions, surgeries, medicines and allergies. Many practices combine the two in one packet.

What are KOOS, HOOS and Oswestry questionnaires?

They are scored questionnaires that patients fill in themselves. KOOS covers knee pain, symptoms, daily function, sport and quality of life; HOOS does the same for the hip. The Oswestry questionnaire measures how back pain limits daily activities. Practices repeat them over time to track progress.

Why do injury cases need their own forms?

Sports injuries, auto accidents and work injuries need details a routine visit does not: how and where the injury happened, who else is involved, and for work or auto cases, claim and employer details. Separate forms keep these questions out of every other visit.

How does AI Doctor help an orthopedic practice?

AI Doctor, a real-time AI healthcare assistant avatar, asks the intake questions as a conversation, in the patient's own language. It collects the answers for the team to review before the visit, so staff spend less time chasing missing details.

Which form should a new orthopedic practice start with?

Start with the orthopedic new patient form and the medical history form, since every patient needs them. Add a pain diagram next, then the forms for the body areas and surgeries your practice sees most.

Does AI Doctor give medical advice?

No. It collects information for the care team. It doesn't diagnose, triage, or recommend treatment.

Can patient answers stay on our own servers?

Yes. RAVATAR offers on-prem deployment: the platform can run inside your own perimeter, so patient answers stay there. RAVATAR complies with GDPR and the EU AI Act.
