Podiatry intake forms: FAQ

What intake forms does a podiatry practice use?

Most start with a new patient form: contact and insurance details, medical history, medications, and the foot or ankle problem behind the visit. Practices then add a consent to treat form and forms for common complaints, such as heel pain, bunions, ingrown toenails, or diabetic foot care.

Why do podiatry forms ask about diabetes and circulation?

Diabetes, nerve damage, and poor circulation change how a foot problem is assessed and treated, and they raise the risk of foot ulcers. The American Diabetes Association recommends a comprehensive foot evaluation at least once a year for people with diabetes.

What are foot outcome questionnaires like the FFI or MOXFQ?

They are standard questionnaires the patient fills in to score pain, function, and daily activity. Practices often use them before treatment and again later to track change. The Manchester-Oxford Foot Questionnaire, for example, has 16 items on walking and standing, pain, and social interaction.

How are pediatric podiatry forms different?

A parent or guardian usually fills them in. Besides the main concern, such as flat feet, toe walking, or heel pain, they ask about walking milestones, activity and sports, shoes, and any family history of foot problems.

How does AI Doctor help a podiatry practice?

AI Doctor, a real-time AI healthcare assistant avatar, asks the intake questions as a conversation, in the patient's own language. It collects the answers for the podiatry team to review before the visit.

Which form should a new podiatry practice start with?

Start with the new patient form and the consent to treat form, since every patient needs them. Then add the complaint forms that match your most common visits, such as heel pain, ingrown toenails, or diabetic foot checks.

Does AI Doctor give medical advice?

No. It collects information for the care team. It doesn't diagnose, triage, or recommend treatment.

Can patient answers stay on our own servers?

Yes. RAVATAR offers on-prem deployment: the platform can run inside your own perimeter, so patient answers stay there. RAVATAR complies with GDPR and the EU AI Act.
