Urology intake forms: FAQ
What intake forms does a urology practice use?
Most start with a new patient form and a medical history. Then they add symptom questionnaires for the main complaints, such as urinary symptoms, incontinence, kidney stones, erectile dysfunction or blood in the urine, plus a pre-op form for procedures.
What is the IPSS questionnaire?
The International Prostate Symptom Score asks seven questions about urinary symptoms such as frequency, urgency, weak stream and waking at night to urinate. Each is scored 0 to 5, for a total of 0 to 35. An eighth question asks about quality of life.
What is a bladder diary?
A bladder diary is a record the patient keeps at home for a few days. It logs the time and amount of drinks, each trip to the toilet and any leaks. It gives the team a clearer picture than memory alone.
Are urology forms different for men and women?
Many questions are shared, such as urinary symptoms, infections and stones. Female urology forms add pelvic floor, incontinence and pregnancy history. Male forms add prostate symptoms, erections and fertility.
How does AI Doctor help a urology practice?
AI Doctor, a real-time AI healthcare assistant avatar, asks the intake questions as a conversation, in the patient's own language. It collects the answers for the urology team to review before the visit.
Which form should a new urology practice start with?
Start with a new patient form and a medical history. Then add the symptom questionnaires your patients need most often, such as the IPSS for prostate symptoms or an incontinence questionnaire.
Does AI Doctor give medical advice?
No. It collects information for the care team. It doesn't diagnose, triage, or recommend treatment.
Can patient answers stay on our own servers?
Yes. RAVATAR offers on-prem deployment: the platform can run inside your own perimeter, so patient answers stay there. RAVATAR complies with GDPR and the EU AI Act.
