4 formsGeneral intake forms: FAQ
What forms does a new patient fill in?
Most practices use three: a patient registration form, a medical history form, and a consent form. Many add a form for their own field, such as a massage intake form or a dental health history.
How is a registration form different from a medical history form?
The registration form records who the patient is: name, date of birth, contact details, insurance, and emergency contact. The medical history form records their health: conditions, medications, allergies, past surgeries, and family history. Front desk staff use the first; clinicians read the second.
How is a consent form different from the other two?
A consent form records the patient's agreement to a specific treatment or procedure. Informed consent comes from a conversation in which the clinician explains the treatment, its risks and benefits, and the alternatives, including saying no. The signed form documents that decision.
How often should patients update these forms?
Whenever something changes. Address and insurance change, and so do medications and diagnoses. Many practices ask returning patients to review and confirm their details at each visit instead of filling in a new form.
What does a massage intake form ask?
It asks where the client feels pain or tension, the pressure they prefer, and the goal of the session. It also asks about health conditions, medications, recent injuries or surgeries, and pregnancy, so the therapist can adjust or avoid certain techniques.
How does AI Doctor help with new patient forms?
AI Doctor, a real-time AI healthcare assistant avatar, asks the intake questions as a conversation, in the patient's own language. It collects the answers for the practice team to review before the visit.
Does AI Doctor give medical advice?
No. It collects information for the care team. It doesn't diagnose, triage, or recommend treatment.
Can patient answers stay on our own servers?
Yes. RAVATAR offers on-prem deployment: the platform can run inside your own perimeter, so patient answers stay there. RAVATAR complies with GDPR and the EU AI Act.
